Important Forms to fax
Payment is expected at time of visit. Please tell Marilyn Spenadel, LCPC if you would like a receipt to give to your insurance company or for your records. If you're a new client, please complete the following forms and fax, (240) 622-6220, to the office or bring them to your first therapy session. Email Marilyn before they're faxed.

If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), complete this form to authorize release of psychotherapy information:
